Welcome to Vine Surgery

We are pleased to accept registration requests from patients who are living within the area indicated on the map below for both our practices:

Please note that the map below is a window into another website which we have no control over. You may find, particularly on mobile devices, that you may need to drag the map left to enter details and click.


New Patient Registrations
 
If you live here, or are moving into this area, you are welcome to apply to register with our practice.
 
To register with us you can either visit the practice and we will give you a registration pack. The pack contains various forms which you can complete at the surgery or take home.

Important: Proof of your new address will be required. This can be in the form of a utility bill, bank statement or driving licence etc. Please bring this with you when you visit to complete your registration.

If you have a medical card please bring it with you to the surgery as will assist us in transferring your existing medical records (though cards have now been phased out, so many patients will not have one).

When you register with the practice you can request an appointment with the practice nurse for a new patient health check. This allows any significant health problems or family history to be recorded prior to the receipt of your medical record and allows you to discuss any issues or concerns you may have.

You will also be given a selection of further information and a practice leaflet which gives you all the information about the two separate GP Practices, their services provided, and the surgery opening times.

 


Visitors to the Practice
